From what I've seen, nothing about the game's mechanics change when you switch between Realistic and Simple modes. The only thing that changes is the on-screen appearance.

Some players prefer Simple because of increased visibility of peripheral vision. Others prefer realistic, like me. I like the feeling of being in an actual cockpit.
