I agree, I just had a conversation with this person in a game. I calmly stated he should change his name, he said he couldn't because he paid ? I told him that he could by going to the website and use the tools tab. Hopefully he changes his name, he seemed receptive to my input

Sadly, this kind of behavior is adopted by many players.
I still think that the only possible and effective workaround would be denying people from changing in-game names, and limiting these names to a single word, with no spaces, no special characters, and to a limited number of digits/characters.
And after a name is chosen by a player, the moderators should be notified of this name, and should have some tools to do the necessary actions to deny bad names.
I know that this would be resource-and-human-material-consuming, but I see no alternative methods...

Try to explain why they should change their name first, do the same with spawnkillers, teamkillers, etc. It works sometimes.

I agree with calvin. They might not understand y. One reason might be that there are kids playing.
